Brianna Titone

Brianna grew up in a small town in the Hudson Valley of New York. She had hard working parents who both worked to provide for her and her brother. They taught her the value of money, how to save, and how to treat other people. She grew up very close to her grandparents and learned the value of wise elders and their experiences. She started working at age 14 to pay for the things she wanted and always had a job (or three) from Middle School through College. She was very fortunate and thankful for her parents to pay for her way through college at the local state university. This was something that she did not take for granted.

Healthcare

Colorado Indigent Care Program - provides discounted health care services to low-income people and families.  Discounted health care services are provided by Colorado hospitals and clinics that participate in the CICP. Includes information on eligibility requirements and how to apply.

Metro Community Provider Network - Clinics all over Jefferson County including Jefferson Center Health Homes and the Kids and Teen clinics at Stein Elementary and Jefferson High School. MCPN serves patients with Medicaid, Medicare, CHP+ and sliding scale. Spanish speaking services available. Contact Navigation to be referred to a health home located at Jefferson Center.

Carin’ Clinic - This is a low cost health center serving uninsured children from birth to 18 years of age. ï‚· Carin' Clinic serves children that have Medicaid, CHP+, and children with no health insurance ï‚· Jefferson County residents only. Mon & Fri (9am-2pm), Tues – Thur (12pm-7pm) ï‚· Closed on Weds and Fridays in June and July

The Action Center – Health Navigators goal is to give clients a continuum of services to meet their current needs and the means to become self-sufficient. It is through the following programs that The Action Center gives help, provides hope and builds dignity while breaking the cycle of poverty.

Jefferson County Public Health Department Clinics: Services: Community Health Services, WIC Program, Immunizations, HIV Counseling and Testing, Family Planning, Nutrition Services, STD Clinic, Women’s wellness exams, Pap smears, Vasectomy Services, and other community health services like tobacco cessation.

Planned Parenthood - Offers women’s health services, STD testing, birth control services, pregnancy testing/options/services, Men’s sexual health services
